Gawande is an excellent writer who deals with end-of-life situations with wisdom, sensitivity, and compassion. Atul’s book has a wonderful rational elegance. This is more than that. First, he shows us how difficult it is to age gracefully in the modern age. One of his key insights is that the usual hierarchy of needs is inverted. People care more about having meaning, purpose, and control over their lives than about their safety. The need for connection and self-determination is paramount. This first part also shows how easily institutions and innovations aimed at improving the aging process can fail. The second part moves from aging to death. This section is mostly about the journeys of a few individuals and the tough conversations that shaped their last years, months, and days. We see how difficult and important it is to make connections between personal values and the technological options that health care offers. We believe that even something as vague as “don’t do anything dangerous” can be a useful guideline for surgeons. We are also taken to the later years of the author’s father. This story is personal and humble. Not only are we mortal, we are also human. An important book for us to read, discuss with our loved ones, and take action. This book deserves to be read by a wide audience because we are all readers.
